Hyperlinking to a file in an org-mode file using tramp does not work as
expected. Consider this file:

This link works
[[/sudo:root@localhost:/etc/hosts]]
this link doesn't
[[/sudo::/etc/hosts]]
*****************
The first one starts tramp as expected but the second version gives the
following errors:
File not found and directory write-protected
org-open-file: No match for fuzzy expression: /etc/hosts
Is this a bug or am I making a mistake of some kind.
Adding the ""file:"" header to the link does not help.

The syntax '::', in file links, is used for search options.
That is, your short version is read as:
path = "/sudo"
=> File not found and directory write-protected
search options = "/etc/hosts"
=> org-open-file: No match for fuzzy expression: /etc/hosts

You have to stick with the long version.
Note that org allows you to easily define link abbreviations.
If you write this in the document:
#+LINK: sudo /sudo:root@localhost:
You may then write your 'sudo' links like this:
[[sudo:/etc/hosts]]
