I was exporting my notes via LaTeX.  Importantly, I have ALT_TITLE set
on every note, automatically, to make Texinfo use the URLs I want.  I
found the LaTeX output to be broken, so I created a minimal example and
reported the bug here.

[Would I expect the notes with ALT_TITLE to show up in the table of
contents?  Probably so, as that is what Org does for HTML, but I will
let the experts on Org decide on that.]
