Because the keystroke is incomplete.

It needs to be one of these:

C-c C-e h h
C-c C-e h H
C-c C-e h o

Yes.

Because with no .emacs you are relying on the builtin Emacs version of
Org, which is certainly 7.8.11.  Try M-x org-version RET in both cases
and you will see the difference.
