Thanks for the pointer!  Installing GNU texinfo and making sure it's before the system one in my path fixed the problem.

Try the Gnu version of “makeinfo” (may be available as “gmakeinfo”,
maybe you'll have to install it).

I use “gmakeinfo” with the following lines added to my local.mk

# How to create the info files from the texinfo file
MAKEINFO = gmakeinfo

# How to create the HTML file
TEXI2HTML = gmakeinfo --html --number-sections
