The behavior I see is that I always need to start with a top-level
heading. For your structure below, I can go to a, b, or c no matter
where I am in the document, but in order to go to d, I need to do b/d.
If you really can't go to b (a top level node), then something is messed
up. If the problem is just that you can't *start* completion at a
non-top-level node, then that's the way it's supposed to work.

What version of org are you using? Ido completion should certainly work
with that variable set. Have you successfully activated ido in other
contexts?
