The entities errors go away, if the rest of the file is valid.

But one more:

To be valid, this link here:

<a href="x.php?a=b&c=d">x.php?a=b&c=d</a>

should be:

<a href="x.php%3Fa%3Db%26c%3Dd">x.php?a=b&amp;c=d</a>
                   ^                        ^
urlencoded--------'              entities--'
